In recent days oil market has witnessed surge in global oil price. Crude oil went to a two-month high on concern that U.S. refiners will fail to produce enough gasoline to keep up with peak demand this summer.
Short fall in supply of oil from Nigeria and uncertainty on Iranian nuclear issues is already keeping buyers of oil nervous. Delivery in May future hit $68 per barrel, a 20 pct jump from last years $56.5 per barrel, only $2.85 from August 2005 high’s of $70.85.
Since last 3 years oil market has been witnessing a substantial rise in the average price of oil. Last week number of oil analyst and agencies have one again raised their 2006 average forecast price to $63.
US Energy department Data reported that refineries are operating around 86 pct of their capacity. Analyst estimates that during same period last year, plants utilised 94 percent of their capacity. Not to forget that February, March & April are also crucial as all the maintenance work is done during these months.
There is difference of opinion as experts differ on whether the current soaring oil demand will outstrip the current supplies, and how quickly.
But for oil watchers, what could be more concerning is that if the current surging demand from China and India persists then Saudi Arabia, which has a known 25% global oil reserve, may see its oil reserves dwindle in twenty years time. The country has the ability to produce 15 million barrels per day. Middle Eastern Oil analyst is of view that if Saudi Arabia produces 15 mbp, the lifespan of Saudi Arabia’s proven oil reserve of 260 billion barrel, 100 billion has already been used and therefore the reserves can be used in our lifetimes.
Meanwhile, last year’s impact of five major hurricane hitting United States of America still has the biting effect on the oil industry. Coastal oil refineries are still fighting to deliver maximum production.
Developing oil sands or natural gas-based diesel fuel is slower and more expensive proposition, though researchers are making every effort to produce an alternate to counter oil price.
US President George W Bush in his one of his State Union address in February, called for intense effort to develop more efficient fuel sources. The US Energy Department and the Agriculture Department spend tens of millions of dollars every year on biomass-based energy research and development. This is in addition to the billions of dollars. More than $4 billion was spent in 2004.The U.S. provides in subsidies for the production of corn, from which most domestically produce ethanol is derived.
Considering how ethanol is produced, corn or sugarcane is grown, harvested and delivered to an ethanol plant. Growing and harvesting the corn and heating the reheating the fermented corn of sugarcane to produce ethanol of a high quality to replace some of the gasoline in car requires enormous amount of energy.
According to researchers, it was found that it takes 30 pct more energy to top make ethanol from corn. Wood biomass takes 55 pct more energy. Swiss grass takes about 50 pct. Ethanol is just highly uneconomical product in the West, as compared in developing countries, also due to low labour wages. It also contributes to air pollution. Cars running on gasoline containing ethanol produce more air pollution than cars running gasoline alone.
Another research work on Pig manure is underway. One pig produces 10 pounds of manure to yield up to 21 gallon of crude oil. Hence, it is estimated manure from America’s 60 million pigs could produce 50 million barrels of oil a year. Framers can earn $10 per pig from manure.
There are all very expensive propositions. The researchers would continue to search for oil alternate, but substitute for oil may still be far away. With current pace of global growth, thirst for economic boom and demand incurring due to population explosion is unending.
I have very few reasons to believe that oil prices will fall to USD 50 per barrel and would rather like to argue that we would continue to see higher oil price trend. Without which search for new oil find could not be met due to high exploration cost. Oil price could also be kept high intentionally, to give investors incentive to explore oil and to developed alternate fuel find which requires billions of Dollars. With growing annual demand for 2 million barrel per day, most of it coming from Asia, one single event that disrupts oil production could send prices sky rocketing. Current demand for global oil is 84 million barrel per day. I expect the oil to trade in a USD 75-80 range in a short span of time. Not long ago talking of oil price averaging USD 60 was a sin. According to Wikipedia, Jet lag, also jetlag or jet-lag, is a physiological condition which is a consequence of alterations to the circadian rhythm. Such alterations result from shift work, daylight saving time, altered day length, or as the name implies, transmeridian travel as on a jet plane. They are known as desynchronosis, dysrhythmia, dyschrony, jet lag, or jet syndrome. The condition is generally believed to be the result of disruption of the «light/dark» cycle that entrains the body`s circadian rhythm. It can be exacerbated by environmental factors.

In the September Issue of Shape magazine, researcher Russel J. Reiter, suggests that eating a handful of dried tart cherries, every night of the trip, half an hour before sleep, could provide a good nights rest. He opines that melatonin presented in the dried tart cherries helps the body to regulate the sleep cycle.

Diego Golombek and colleagues at Quilmes National University in Argentina claim that viagra can counter the effects of jet lag in hamsters. Although, not too many people are ready to pop this pill for jet lag that is most notibly used for erectile disfunction.

Alina Larson of Positive Thinking reports one should get plenty of rest because adequate sleep reduces the effects of jet lag, another stress on the immune system.

Professor of psychiatry Norman E. Rosenthal suggests taking melatonin a half hour before bedtime while traveling. Director of the Sleep Center in Edinburgh, Scotland Chris Idzikowsi comments on the importance of the exposure to light to the body`s internal clock. Ronald D. Novak, a circadian rhythm expert at Case Western University School of Medicine in Cleveland, Ohio, recommends eating protein for breakfast and carbohydrates for dinner.

These are just a few of the many remedies for Jet Lag. However they all have one thing in common, get plenty of rest.

Implementing Change In The Healthcare System

For many of us change is a difficult process. In organizations like healthcare it seems to advance at a snail’s pace sometimes. There is a need for change in healthcare, most agree, though we would be hard pressed to agree upon the changes needed. One incentive for change is pay-for-performance programs now beginning in several areas. I would like to describe a couple that affect primary care physicians and then give a few suggestions as to how to adopt changes to take advantage of these programs. Even if you are not in a primary care physician program, the methods suggested for change will be helpful, I believe.

In 2006 Medicare plans to institute a pay-for-performance program at the primary care physician level. Right now a model is being tested and seems to be doing quite well. In several states Blue Cross Blue Shield organizations are testing pay-for- performance programs. Here in West Michigan, Priority Health, a healthcare insurer, has promoted such a program for over five years. How does this work, you may ask? Priority Health, for example, funds the program for each of its patients a set amount. Doctors who meet a requirement of the program for a patient are rewarded with extra money for that patient. Hence, with many patients the income for the practice can be boosted considerably. The fact is that many are not rising to the opportunity. With planned cuts in Medicare reimbursements over the next few years, this source of income cannot be ignored! Healthcare programs need to change, no matter how difficult.

The impetus for change should rest with the leadership of an organization, although the change should not be the sole responsibility of the leaders. Representatives from all parts of the organization should be involved. Once the need for change in a process is agreed upon, either because of extra revenue from pay-for-performance programs or other agents or data that positively affect the bottom line, leaders should convene a task force to plan the change. With input from all, leaders should map the process as it currently exits and then should make a new map of how they would like it to be to incorporate the positive changes. The new procedure should be standardized for all to adopt.

How do you go about adopting these changes on a daily basis? This is probably the hardest part. Because humans learn in a variety of ways, it will take a variety of ways to implement the changes. The implementation of the changes should be based upon the learning styles of the individuals involved. Let me provide an example using the Medicare program. A patient who enters the Medicare program is entitled to a paid initial physical. A primary care physician should take advantage of this. Many don’t. If I were the manager of such an office, I would remind my staff who set up appointments to be aware of this fact. I would remind them at regular staff meetings. I would post visible reminders in the office. I might even have a message flashed on the computer screen once in a while. Then I would review the appointments of patients who have become Medicare qualified and see how many had their initial physical or were booked for it. I would adjust my methods to remind staff of the need for such examinations and continue to improve on this until the office achieves 100% compliance with the goal.

Booking the exam is not the only needed change. Doctors who perform the physical must accomplish examination details and actions laid out by Medicare. Hence, to be paid for the exam, each doctor must adhere to the exam details. I would help the doctors accomplish this in a variety of ways, depending on the doctor’s learning style. For example, a checklist of the exam details might be included in the patient history folder when the exam is performed. That way the doctor will not miss any steps. As the leader of the change, I would check with billing to see that all the steps were performed and adapt new approaches or reinforce existing ones to see that the changes are accomplished 100% of the time.

Changes such as these should be a part of a continuous quality improvement program at every healthcare provider organization. Let me quickly review the most important steps. First leaders should identify the changes needed. Then, the leaders should convene a committee of all affected staff to develop how to accomplish the change. Once the staff agrees upon the approach, the leaders should develop ways to implement the change on a daily basis adopting methods that incorporate learning styles of affected individuals. Then, they should continually analyze the progress of the changes and make necessary adjustments until the goals are accomplished. They should then audit the changes occasionally to be sure that the organization doesn’t fall back into old habits.

I believe that adopting such a change process will dramatically help at your site. You will see savings in time, increased patient or client health and satisfaction, as well as an improved bottom line!
